Editorial policy

Each listing on this site starts from a public record, not from what a facility says about itself. This page explains where the facts come from and how we correct them.

Where the listings come from

  • Florida assisted living: the licence roll of the Agency for Health Care Administration (AHCA), with its facility profiles and its searches for Medicaid, memory care and bed types.
  • California assisted living: the Community Care Licensing Division of the Department of Social Services, and the Department of Health Care Services list of Assisted Living Waiver facilities.
  • Nursing homes in every state: the CMS Provider Information file, October 2026 release, with star ratings, inspections and fines as CMS publishes them.

We show the agency's own words for licences, citations and ratings, and link each listing to its record. We don't turn an inspection into a verdict, and we don't blend star ratings with anything else.

How the guides are written

Guides cite the agency or law behind each rule, such as Medicare.gov, Medicaid.gov, the state Medicaid agency or the state code. Cost figures are medians from the CareScout Cost of Care Survey 2025 and are labelled as medians, not prices. We don't quote a home's rate unless the home published it.

Corrections

If a listing is wrong, write to contact@assistedlivingaccess.com with the facility's name and what's wrong. We check it against the record and fix our copy. If the record itself is wrong, we'll tell you which agency to contact.
